We are seeking a compassionate and dedicated Social Worker to join the Adult Community Team. As a vital member of the team, you will be playing a key role in providing essential support and services to adults in the community, empowering them to lead fulfilling lives and promoting their overall well-being.
Responsibilities include, but are not limited to:
- Conducting comprehensive assessments of individuals' social care needs, considering physical, emotional, and environmental factors.
- Developing and implementing tailored care plans in collaboration with service users, promoting independence and resilience.
- Providing crisis intervention and support during challenging situations, working closely with other professionals and agencies.
- Working collaboratively with healthcare professionals, local authorities, and community organisations to provide holistic care.
- Attending multi-disciplinary meetings, contributing valuable insights and expertise to enhance the overall care and support network.
- Fostering positive relationships with external agencies to strengthen the support available to service users.
- Maintaining accurate and up-to-date records of assessments, interventions, and service user progress.
- Preparing reports for case conferences, reviews, and legal proceedings as required, ensuring compliance with data protection and confidentiality policies.
- Qualified Social Worker with a recognised Social Work degree.
- Previous experience in adult social care and community-based settings.
- Strong knowledge of relevant legislation, policies, and procedures.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to maintaining professional boundaries and promoting ethical practice.
