Community Care Officer

Job description

Responsibilities include, but are not limited to:

  • Dealing with telephone, written, emailed enquiries and respond appropriately to customer enquiries, complaints, requests for information and service requests.
  • Assisting customers in their understanding and completion of relevant document forms, applications and registrations, over the phone, including where appropriate addressing their specific needs.
  • Processing referrals and applying agreed eligibility criteria.
  • Recording detailed referrals, specifying clients needs.
  • Working within agreed procedures.
  • Providing information and advice to members of the public from diverse backgrounds.
  • Liaising with other departmental and non-departmental services and partner agencies as required.
  • Dealing with emails, letters and other aspects of First Response's routine administration on a rota basis.
  • Providing support, assistance, mentoring to new / temporary staff and work experience placements.
  • Operating professionally both as an individual and as a member of a team, contributing ideas, suggestions and feedback to managers as appropriate.

Requirements:

  • Relevant experience working with people with disabilities.
  • Ability to write letters and reports.
  • Ability to communicate effectively at all levels.
  • Ability to take responsibility for and organise own workload.
  • Awareness of relevant NHS and Social Care legislation and Health and Safety art Work Act.
  • Awareness of costs and effective use of resources.
  • Enhanced DBS, dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
  • Willingness to complete an enhanced DBS if you do not currently hold one.
