The Recovery Coordinator plays a key role in supporting individuals affected by substance misuse and related issues to achieve sustained recovery. You will provide person-centred interventions, coordinate care plans, and work collaboratively with partner agencies to improve health, wellbeing, and social outcomes.
Manage a caseload of service users with varying needs (e.g. substance misuse, mental health, housing, offending history).
Complete comprehensive assessments and develop tailored recovery care plans.
Deliver structured interventions, including harm reduction, relapse prevention, and psychosocial support.
Regularly review and update recovery plans in collaboration with service users.
Identify and manage risks, including safeguarding concerns for adults and children.
Follow organisational and local safeguarding procedures.
Maintain accurate and up-to-date risk assessments and case records.
Work closely with external agencies such as:
Healthcare providers
Housing services
Criminal justice agencies
Social services
Attend and contribute to multi-agency meetings.
Build trusting, professional relationships with service users.
Use motivational interviewing and recovery-focused approaches.
Support individuals to access education, employment, and community resources.
Maintain accurate records using case management systems.
Ensure compliance with data protection, confidentiality, and organisational policies.
Meet performance targets and service KPIs.
Provide advice on physical and mental health, including:
Safer substance use
Blood-borne virus prevention
Smoking cessation
Support access to treatment services (e.g. prescribing, detox, rehab).
Experience working with vulnerable individuals (e.g. substance misuse, homelessness, mental health).
Strong communication and interpersonal skills.
Ability to manage a caseload and prioritise workload effectively.
Knowledge of safeguarding and risk management.
IT literacy and experience with case recording systems.
Relevant qualification (e.g. Health & Social Care, Counselling, or equivalent).
Experience in substance misuse or recovery services.
Knowledge of Calderdale local services and pathways.
