Adults Social Worker

Job purpose:

• Undertaking cases independently and demonstrating an ability to adapt the approach from first principles to deal with complex or unusual cases, assessing and managing risk appropriately.
• Demonstrating the ability to deal with complex cases.
• Working under pressure, managing complex relationships, high levels of risk to service users, safeguarding vulnerable adults and making judgements about the need for compulsory action.

Responsibilities include, but are not limited to:

• Undertaking a range of community care and safeguarding assessments, in accordance with relevant statutory requirements, identifying risks, needs and options.
• Developing and implementing care plans.
• Working with individuals and their families, carers and communities to help them make informed choices and decisions.
• Managing an allocated case load of clients, regularly monitoring, reviewing and evaluating changes in clients needs.
• Developing and maintaining effective relationships with clients, their families and carers.
• Undertaking post qualifying training.
• Updating case notes and other records.

Requirements:

• Social Work Degree.
• At least two years post qualifying experience as a Social Worker, assessing and managing risks.
• Previous experience of working with older people and their families / carers.
• Proven ability to undertake assessment of older people and their families / carers.
• To value diversity and use positive terminology referring to service users.
• Commitment to a needs led approach to maintaining maximum independence and to offering people opportunities to remain in their own homes.
• Enhanced DBS, dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
• Willingness to complete an enhanced DBS if you do not currently hold one.
